About The Position

The Facility Nurse provides essential medical and health services to children served at The Centers H.O.P.E. Campus, delivering trauma-informed, compassionate care to multi-system youth navigating complex behavioral health, developmental disability, juvenile justice, and child welfare challenges. Our goal is to create a lasting impact through positive interactions that focus on assisting our clients with the tools needed to overcome barriers—to assist with confidence building and emotional regulation through transparency and active listening without judgment. The Centers' H.O.P.E. Campus (Healing, Optimism, Partnership, and Enrichment) serves as a hub for child welfare transformation in Cuyahoga County. We provide emergency response, stabilization, and residential programming for youth facing complex placement challenges through trauma-informed care and cross-system collaboration. Requirements

  • Associate degree in nursing (ASN) or Bachelor of Science in Nursing (BSN)
  • Active Registered Nurse (RN) license in the State of Ohio
  • Minimum two (2) years of experience in pediatric and/or public health nursing
  • OR an equivalent combination of education, training, and experience

Responsibilities

  • Conduct comprehensive physical health assessments
  • Provide well-child care services
  • Administer first aid and emergency interventions
  • Respond to and implement physician's orders
  • Dispense, administer, and supervise medication management
  • Assist residents with self-administration of medications
  • Monitor for medication side effects and adverse reactions
  • Deliver medical interventions as clinically indicated
  • Provide medication education to clients and families/caregivers
  • Deliver health education instruction to residents
  • Train agency staff on universal precautions and communicable disease protocols
  • Conduct staff training on medication dispensing procedures
  • Serve as resource person on pediatric health issues
  • Maintain accurate, confidential medical and dental records in compliance with HIPAA
  • Document immunizations, health findings, and relevant health data
  • Schedule medical and dental appointments
  • Transport children to appointments as needed
  • Obtain medication approvals and order medical supplies as directed
  • Participate in clinical and treatment team meetings
  • Coordinate medical services with interdisciplinary treatment providers
  • Collaborate with community partners and child welfare systems
