About The Position

The Residential Registered Nurse (RN) plays a critical leadership role in ensuring the health, safety, and well-being of children, youth, and young adults residing in Gladney’s licensed Group Residential Operation (GRO). This position provides direct clinical oversight of medical services, including the administration and monitoring of psychotropic and other prescribed medications. The RN is scheduled to be on-site weekly for thirty (30) hours to provide direct clinical support, conduct health assessments, review medication protocols, and collaborate with residential and clinical teams. In addition to scheduled on-site hours, the RN remains accessible 24 hours a day, 7 days a week via phone or secure video conference to direct and oversee medication administration and respond to urgent clinical concerns. The nurse works closely with multidisciplinary staff supporting individuals receiving substance use treatment services, mental and behavioral health services, and intellectual or developmental disability and Autism spectrum disorder treatment services. This role requires strong clinical judgment and the ability to assess patient health status, identify emerging needs, and recommend appropriate medical interventions. The Registered Nurse ensures compliance with all licensing regulations, medication management standards, and best practices while promoting high-quality, trauma-informed, person-centered care within a residential treatment setting.

Requirements

  • Associate’s or Bachelor’s degree in Nursing from an accredited Registered Nurse (RN) program required.
  • Must have successfully passed the NCLEX-RN® examination.
  • Must hold a current, active, and unrestricted Registered Nurse (RN) license in the State of Texas (or be eligible for licensure by endorsement).
  • Must have practiced nursing or have passed the NCLEX-RN® examination within the four (4) years preceding the date of application.

Responsibilities

  • Maintain accurate and detailed medical records, documentation, and reports regarding residents’ health status and medical needs.
  • Monitor, assess, document, and report symptoms, behavioral observations, and changes in residents’ physical or mental conditions.
  • Record and track residents’ medical information, including vital signs and clinical observations.
  • Evaluate resident responses to treatment and modify care plans as clinically indicated.
  • Collaborate with multidisciplinary client care team members to assess, plan, implement, and evaluate individualized care plans.
  • Oversee and monitor all aspects of resident health, including nutrition, medication adherence, and physical activity.
  • Prepare residents for medical examinations and treatments and provide clinical support as needed.
  • Conduct health assessments to identify potential medical, safety, or risk concerns.
  • Provide education and training to the client care team on health-related topics, disease prevention, medication management, and wellness initiatives.
  • Perform physical assessments, make preliminary clinical evaluations, and provide necessary treatment or stabilization prior to hospital transfer when appropriate.
  • Prescribe or recommend medications, medical devices, or therapeutic interventions in accordance with scope of practice and physician directives.
  • Coordinate referrals to specialized medical providers, community health resources, or external agencies as appropriate.
  • Partner with the client care team to design and implement programs that promote the overall health and well-being of residents.
  • Administer medications and monitor residents for adverse reactions or side effects.
  • Oversee medication management processes and complete all documentation required to ensure compliance with regulatory and licensing standards.
  • Perform other duties and special projects assigned to support organizational objectives and enhance operational efficiency.
