The Residential Program Coordinator enhances the quality of life for a caseload of people with intellectual or developmental disabilities who live in a host home setting. Supports people receiving services in creating the lives they desire by facilitating successful Host Home partnerships and settings. Acts as liaison between individuals receiving services, Host Home Providers, family members, and team members to ensure the pursuit of the individual’s goals and choices.

  • Ensures that the self-expression, self-worth, self-reliance, and decision making of individuals on caseload is supported and respected through Host Home services.
  • Fosters collaboration with individuals receiving services, their loved ones, the Host Home providers, and their support team.
  • Ensures that training, resources, and work environment are provided to support individuals’ receiving services in pursuing their goals, activities of interest, and social interactions.
  • Ensures implementation of the Annual Plans and Individual Service and Support Plans (ISSPs) for people receiving services.
  • Continually evaluates and assesses the programs’ strengths and weaknesses to improve services and supports using a person-centered approach.
  • Supports a culture where individuals receiving services are healthy, safe, and valued by others.
  • Models and teaches respect for others, including individuals in services, Host Home Providers, guardians and loved ones, and team members.
  • Ensures the safety and well-being of individuals receiving services by providing oversight, guidance, and resources to Host Home Providers.
  • Ensures that the preferences of individuals living in Host Homes are considered, and that homes are cleaned and maintained according to standards.
  • Conducts required assessments of individuals receiving services and ensures prompt follow-up when deficiencies or areas for improvement are found.
  • Responsible for compliance with PPCH policies, and the rules and regulations of State and Federal oversight agencies.
  • Ensures bookkeeping, and recordkeeping meets standards of PPCH and State and Federal agencies.
  • Oversight of and responsibility for current and complete Host Home tracking and documentation, including health-related services.
  • Conducts regular visits to Host Homes to ensure that individuals receiving services are safe, healthy, satisfied with services, and that the setting meets all quality standards.
  • Ensures Host Home Providers and Respite Providers have completed required trainings, including annual refreshers, according to established timelines.
  • Maintains knowledge and understanding of PPCH and other regulating agencies’ policies, procedures, rules, regulations, and guidelines.
  • High school diploma or equivalent required.
  • Bachelor’s degree preferred.
  • One or more years of work experience in the field of intellectual or developmental disabilities required.
  • One or more years of experience working in the Colorado Developmental Disabilities System is preferred.
  • Excellent written and verbal communication skills.
  • Strong computer skills with proficiency using Microsoft Office is required.
  • Effective listening skills, even when non-traditional communication is used.
  • Ability to lead and manage others.
  • Knowledge of Colorado Developmental Disabilities system, community support systems, and human services is helpful.
  • Knowledge of Social Services and out of home placements for adults with intellectual or developmental disabilities is helpful.
  • Ability to respectfully address significant issues of health and safety while supporting an individual’s choices.
  • Ability to work as part of a comprehensive team and represent PPCH in the community.
  • Skill and ability to constructively receive and provide feedback.
  • Ability and willingness to complete required trainings and learn PPCH operations and systems.
  • Valid driver’s license, and ability to meet PPCH’s driving and insurance requirements.
  • Ability and willingness to respond to emergency situations in Host Homes.
