About The Position

Croyle-Nielsen Therapeutic Associates (C-NTA) is seeking a collaborative and highly- organized Registered Nurse (RN) or Licensed Practical Nurse (LPN) to serve as our Residential Medical Specialist. Moving away from the traditional bedside environment, this is an administrative leadership role designed for someone who excels at the "big picture" and understands that human services is a true calling. The Residential Medical Specialist’s role is to coordinate the continuity of care and coordinate all medical needs for our individuals diagnosed with intellectual/developmental disabilities receiving services within our Residential Department while also while also orchestrating the medical coordination for our Adult Day Program Department. This position oversees the medical services of a large caseload across multiple locations ensuring that the medical needs of our individuals are met with precision and care.

Requirements

  • Active RN or LPN license in good standing. Preferred Bachelor of Science in Nursing degree with an RN license. A minimum of 5+ years’ experience as an LPN or RN
  • Demonstrated ability to manage and coordinate medical services for a large, complex caseload with the flexibility to work across multiple service lines
  • Strong working knowledge of medication systems, regulatory compliance, and documentation standards
  • Exceptional organizational skills with the ability to manage competing deadlines and proven ability to train, lead, and hold others accountable to high standards
  • Strong ownership mentality with the ability to manage a large, fast-paced workload while meeting strict deadlines without sacrificing accuracy or quality
  • Ability to be flexible and work effectively within a 24/7 environment
  • Calm, decisive leadership style in high-pressure situations with a high level of integrity, follow-through, and ownership
  • Leadership capability that can consistently execute. This role is not suited for someone seeking a low-volume or task-based nursing position. Proven leadership abilities with experience supervising a team and a confidence to lead, coach, and hold staff accountable in a fair, consistent, and professional manner

Responsibilities

  • Serve as the primary medical liaison between healthcare providers, individuals served, guardians, and C-NTA staff
  • Stay abreast of all individuals’ medical needs and changes by serving as the lead Clinical Reviewer and Rater for the State HRST (Health Risk Screening Tool) Program, maintaining strict adherence to required timelines and updating all care plans to improve each individual’s overall health goals
  • Oversee and coordinate medical care across multiple locations, ensuring quality, consistency, and compliance by conducting routine audits of medical documentation and medical skills of all staff.
  • Attend specialty medical appointments, review medical documentation, schedule follow-up care, and train staff for implementation of physician orders
  • Serve as trainer for the medication administration program, CPR and First Aid, Bloodborne Pathogens, and all medical protocols. Will create a training routine, train, retrain and hold all staff accountable to all medical standards.
  • Oversee medication administration systems and regulatory compliance of all medical-related needs for the Residential and Adult Day Program departments.
  • Participate in intake reviews to assess medical appropriateness and needs of individuals entering the Residential program for services.
  • Implements and maintains a system to ensure C-NTA is in full compliance with all medical regulations as outlined in PA 6100, 6400 and 2380 regulations, licensing standards, and internal policies and procedures. Ensures all medical components are state-licensing ready at all times.
  • Inventory all medical equipment to ensure all equipment is in full repair and full working order prior to being used by any staff and implements a system to monitor the usage and location of any C-NTA medical equipment.
  • Develop and implement medical-related processes, policies, and documentation training all staff and auditing all usage to ensure full compliance and high-quality standards.
  • Participate in a 24/7 on-call rotation and confidently manage urgent medical situations.
