About The Position

Promotes the philosophy of aging in place while providing support to Residential Living residents related to their general health and wellness needs. Demonstrates resident directed care philosophy while working collaboratively with other team members throughout Landis Homes. Actively engages residents and their support persons in discussion of their needs while providing flexibility and choice. •Provides rapid emergency response and follows emergency procedures. Performs resident assessments, care and treatment in accordance with Pennsylvania Nurse Practice Act. •Provides courtesy visits to new residents, residents transferring to Residential Living from Personal Care, Healthcare and/or hospitals and medical care centers. •Communicates resident emergencies to others as appropriate. •Assists in maintaining resident records in EMR. •Assists residents with follow through related to physician recommendations and orders. •Serves as a resource with residents in providing current information for emergency packets. •Provides oversight to the Emergency Call system in cooperation with the Information Technology and Finance departments. •Completes incident/accident reports and follow-up assessments. •Utilizes devices and follows precautions to promote and maintain a safe working environment. •Works cooperatively and productively with others, including residents, clients, families, consultants and other team members. •Assists in orientation of team members when necessary. •Functions as a resident resource for referral to Landis at Home, consultants, and other support systems. •Assists in maintaining residential living medical equipment

Requirements

  • Current license as an LPN/RN in the Commonwealth of Pennsylvania required.
  • Has and maintains current CPR/AED certification.
  • Prior experience in related role required.
  • Maintains current valid Pennsylvania driver's license.
  • Able to work independently, well-organized, and can multi-task.
  • Proficient in use of computer and related technology required for the role.
  • Possesses good verbal, non-verbal and written communication skills.
