About The Position

As a Case Manager, you will support clients in the Residential System and perform duties in collaboration with a clinician. You will report to the Case Management Supervisor, Residential and you will work onsite with community-based travel.. Requirements

• Requires a HS Diploma/ GED
• Experience with individuals in care systems preferred.
• Must pass physical.

Responsibilities

• Arrange and attend home visits
• Prepare transition plans
• Coordinate Independent Living Services
• Be the primary contact between Schools and Residential
• Coach family visits
• Complete various assessments, including FASP and SPR
• Complete accurate case notes and input the notes into Connections database
• Provide transportation for clients
• Maintain contact with DSS and family
• Function as a contributing member of the interdisciplinary treatment team
• Attend dorm meetings
• Attend treatment review meetings
