About The Position

Responsible for the full client care cycle including assessing, planning, implementing, monitoring, and evaluating actions required to meet the clients’ health and human service needs. Position Description: Underminimalsupervision, coordinate and provide care that is safe,timely, effective, efficient,equitable, and client-centered Maintaincaseload andutilizationbymaintainingregular contact with clients tomonitorand assess progress Handle case assignments, draft service plans, review case progress anddeterminecase closurefor review and/orin consult with supervisor Deliveron-going services to clients which mayincludecrisis intervention, advocacy, medication monitoring, transportationassistance, financial budgeting,housingassistance,and referral and linkage to community resources as needed Assistin helping clients achieve wellness and autonomy through supportive counseling andfacilitatingmultiple care aspects (care coordination, information sharing, etc.) Develop and/or implement individualized client treatment or service plans and ensures compliance with applicable standards and agency policies Document all clinical activities to meet specific program,funderand credentialing entity criteriatimelyand with accuracy Participatein treatment team andassistsin making recommendations for client status changes Mayassistin providing education and/or group curriculum Adhere to professional standards as outlined by protocols,rulesand regulations Complete agency, accrediting body, and funder required training  Other duties as assigned

Requirements

  • Ability to learn case management principles, healthcare management and reimbursement requirements
  • Excellent organizational and time management skills
  • Effective verbal and written communication skills
  • Ability to relate to and interact with a wide variety of people
  • Intermediate level problem solving skills and ability to multi-task while prioritizing
  • Ability to utilize technology including electronic service documentation system
  • Associate’s degree in behavioral science or related field, or equivalent work experience as allowed/required by regulatory bodies
  • Driver License and valid auto insurance

Responsibilities

  • Handle case assignments, draft service plans, review case progress and determine case closure for review and/or in consult with supervisor
  • Deliver on-going services to clients which may include crisis intervention, advocacy, medication monitoring, transportation assistance, financial budgeting, housing assistance, and referral and linkage to community resources as needed
  • Assist in helping clients achieve wellness and autonomy through supportive counseling and facilitating multiple care aspects (care coordination, information sharing, etc.)
  • Develop and/or implement individualized client treatment or service plans and ensures compliance with applicable standards and agency policies
  • Document all clinical activities to meet specific program, funder and credentialing entity criteria timely and with accuracy
  • Participate in treatment team and assists in making recommendations for client status changes
  • Assist in providing education and/or group curriculum
  • Adhere to professional standards as outlined by protocols, rules and regulations
  • Complete agency, accrediting body, and funder required training
  • Other duties as assigned
