Please read the ENTIRE job posting before applying This is an in-office position with a Monday–Friday schedule from 8:00 a.m. to 4:30 p.m. The role also requires participation in an on-call rotation and flexibility to cover open site hours as needed This role operates administratively in a Residential Care Facility (RCF) Setting, which is a type of facility that provides services for people with mental illness and other disabilities (Iowa Health & Human Services). Rather than institutionalizing individuals who have these physical and mental health needs, the RCF settings provides these individuals with the opportunity to live and receive services in a way honoring to them and their abilities. As an RCF Program Supervisor, you will facilitate Habilitation Services with may include HCBS, Daily Habilitation, and all other separately funded programs and services. In this role, you’ll be responsible to: Create and maintain the team culture, which may include: o Coordinate and oversee all aspects of Habilitation Services, including programming for individuals served. o Coordinate and facilitate treatment programming for individuals and groups. o Maintain written documentation of programs. o Communicate with individuals served, guardians, and case managers regarding changes in service provision, limitations, or other concerns regarding the individual. o Schedule and facilitate individuals’ IPP’s. o Complete and maintain an up-to-date social history for individuals assigned to caseload. o Ensure consistency of services for individuals served. o Rotate on-call duties as needed. o Set up, organize, update, and maintain various files and records, including individual records. o Track individual census information and prepare related reports. o Perform administrative duties. Certified Med Aide Duties, which may include: o Accurately administer medications as ordered. o Monitoring and evaluating as needed. o Complete required charting and data collection. o Monitoring individuals for side effects of medications, including behaviors, vital signs, etc. o Complete assessment prior to reporting concerns. o Complete weekly medication change over. o Assemble appointment packets as needed. Demonstrate knowledge, or be willing to learn, and comply with all policies, practices, laws, and rules provided by any governing bodies in the industry. Provide complete, consistent, and accurate documentation of incident reports, investigations, and service documentation. Be an advocate for individuals receiving services to have as much control over their own lives as possible. All individuals should be treated with respect and should feel empowered to live their life as independently as physically possible.
