Opportunity Summary: Origami Rehabilitation is seeking a full-time (40 hours per week, Monday through Friday) compassionate and detail-oriented Residential Care Coordinator to support our clients through thoughtful coordination and communication of care. Working under the guidance of the Clinical Nurse and Residential Manager, you’ll collaborate with an interdisciplinary team to ensure every resident receives the individualized, holistic care they deserve. In this role, you’ll be a steady and trusted presence keeping care plans on track, facilitating effective communication between stakeholders, and advocating for residents in a way that aligns with Origami’s mission, vision, and values.
