About The Position

Origami Rehabilitation is seeking a full-time (40 hours per week, Monday through Friday) compassionate and detail-oriented Residential Care Coordinator to support our clients through thoughtful coordination and communication of care. Working under the guidance of the Clinical Nurse and Residential Manager, you’ll collaborate with an interdisciplinary team to ensure every resident receives the individualized, holistic care they deserve. In this role, you’ll be a steady and trusted presence keeping care plans on track, facilitating effective communication between stakeholders, and advocating for residents in a way that aligns with Origami’s mission, vision, and values.

Requirements

  • Possess a valid State Driver’s License or ability to plan and provide transportation as required
  • Minimum two years of experience in rehabilitation and/or case management
  • Strong interpersonal and organizational skills
  • Ability to manage multiple priorities and maintain confidentiality
  • Basic clerical and technology skills, including Microsoft Office proficiency
  • CPR, First Aid, AED certification (provided by Origami)

Responsibilities

  • Model positive behavior to foster a safe, respectful, and supportive therapeutic environment.
  • Maintain strict confidentiality when handling sensitive information internally and with external partners.
  • Coordinate individualized treatment, transition, and discharge plans with the interdisciplinary team.
  • Proactively communicate with clients, families, providers, and staff to maintain continuity and quality of care.
  • Serve as an ambassador for Origami, delivering excellent customer service and maintaining professional relationships with all internal and external stakeholders.
  • Monitor and support client appointments, external referrals, and community resource coordination.
  • Ensure timely and accurate documentation, including admission paperwork, billing records, and care notes.
  • Participate in clinical discussions, care conferences, and team meetings to support client outcomes.
  • Assist with activities, transitions, and new program initiatives designed to enhance resident experience.
  • Provide on-call supervisor support as part of the established rotation to ensure continuity of strong programming.
