About The Position

Established in 1948, The Mission of ASCEND Mental Wellness is to encourage and assist individuals in discovering pathways to their optimal mental health and wellness, while supporting hope and awareness of well-being in our community. ASCEND is currently looking to fill the position of Restorative Residential Assistant Program Coordinator for our Community Residences, Genesis and Pearl Street. Community Residences: Genesis and Pearl Street Genesis and Pearl Street residences offer a group home environment which provides a high level of support for people in the earliest stages of recovery. The overall goal is to provide short-term, focused skill development in a home-like setting. The first step in treatment is defining and developing a rehabilitation plan that aids a person's movement to the next stage in their recovery. This individualized, highly detailed plan clarifies needs, charts a course of action, and provides the framework to evaluate progress. Services available to residents include crisis intervention, rehabilitation counseling, social and living skills development (money management, personal hygiene, food preparation, meal planning, recreation activities, etc.), symptom and medication management, community integration, and resource development. On duty around the clock, staff work closely with each resident to maximize stability and help in the transition to a less restrictive environment. Long-term assistance is available for people with problems preventing them from living outside a group setting. Individuals must be well-motivated and committed to making a difference in people's lives. Good listening, writing, and verbal skills required. The ideal candidate will be comfortable working independently. Requirements

  • High School Diploma, or equivalent, with at least five years' experience working in the human services field. An Associate's degree in the human services field may be substituted for two years' experience.
  • Valid NYS driver's license acceptable to Agency insurance standards.
  • Must have the ability to work independently, have good problem-solving skills, and have the ability to work as part of a multi-disciplinary team.

Responsibilities

  • Reconcile petty cash weekly.
  • Set up files for new residents and ensure residents complete all necessary paperwork for intake.
  • Archiving of files as necessary.
  • Assess and confirm medication processes, including medication disposal, on a weekly basis.
  • Facilitate the Environmental Tasks List process with the Residence Counselors on a weekly basis.
  • Assure communications of resident hospitalizations to appropriate parties.
  • Assure the scheduling of Relief Residential Counselors for appropriate shifts as needed.
  • Work with supervisor to ensure environmental repairs in a timely manner.
  • Carry a caseload of one to two clients as necessary.
  • Provide additional services for the program as needed.
  • Oversee tracking of resident Service Plan and Functional Assessments.
  • Assist with the provision of orientation of new staff
  • May assist in the coordination of house/community meetings.
  • Coordinate and assist in Rep-Payee services as needed.
  • Maintain a safe environment for all clients and staff.
  • Maintain effective communication with other staff members and supervisor.
  • Preserve human dignity and confidentiality of all clients and act in the best interest of those served through the Agency's programs and services.
  • Assure quality of work at all times.
  • Adhere to all laws and regulations applicable to the Agency's business.
  • Adhere to the Agency's Corporate Compliance Program and established policies and procedures.
  • Positively represent the Agency within the community at all times.
  • Have adequate computer skills, or more refined for specific job titles.
  • Actively participate in scheduled team/staff meetings as scheduled by supervisor or as indicated by the program's services.
  • Actively participate in Agency sponsored staff development and in-service educational programs as deemed necessary by supervisor, including mandatory trainings assigned per job title.
  • Should be compassionate and demonstrate a commitment to helping people with mental illness and/or co-occurring disorders.
