Requirements

• A passion for helping seniors
• A desire to contribute to a positive atmosphere in the community for residents, family & friends, and all team members.
• Ability to communicate effectively in English; both oral and written
• Support services knowledge and insight necessary to assist in the planning for resident care to ensure that the unique needs of each resident is identified and applied as appropriate
• The ability to communicate effectively in English both orally and in writing is essential
• Social work degree is preferred, but the minimum requirement is a high school diploma
• Certificates/ Licenses: A valid driver's license in the state of work is required

Responsibilities

• coordinate a wide range of support services including therapy services, home health, physicians, specialists and others to help the community's residents maximize their quality of life and maintain maximum independence and to liaison with external healthcare settings when needed to ensure the safe and appropriate return of residents
• Document support needs and interdisciplinary notes as appropriate
• Work closely with senior management and the activity team to develop, and participate in, a dynamic activity program focusing on the 6 aspects of the Wellness Wheel including: emotional, occupational, physical, spiritual, intellectual, and social focused programming.
