About The Position

Resident Services Coordinator Inniswood Village Senior Living Campus Westerville, OH Full time Inniswood Village located in Westerville, OH is an independent living, assisted living and memory care community with a team of health care professionals that is unmatched in the area! We are owned and operated by National Church Residences, the nation’s largest provider of affordable senior housing and health care services. We are seeking a Resident Services Coordinator who will share in our vision to advance better living and care for seniors! Qualified candidates for this position offer: Education: High School Diploma or GED. Bachelor’s degree in social work or related field strongly preferred. Experience: Two or more years of experience in social service delivery and/or community program delivery with elderly and/or family population. Must have strong communication, comprehension, computer and interpersonal skills. Skills: Must have working knowledge of Windows, Microsoft Office Suite, and the internet. Licensure: N/A The Resident Services Coordinator assumes the responsibility for assisting new residents with transitioning to community living and with internal resident moves working to promote the highest independence possible in the least restrictive environment. They will continue to serve as the liaison for residents and their family members to manage through any changes that occur during their tenure at the community.

Requirements

  • High School Diploma or GED.
  • Two or more years of experience in social service delivery and/or community program delivery with elderly and/or family population.
  • Must have strong communication, comprehension, computer and interpersonal skills.
  • Must have working knowledge of Windows, Microsoft Office Suite, and the internet.

Responsibilities

  • Assists residents and their family members with the transition process into the community and coordinates internal transfers or moves in the continuum of care as appropriate.
  • Communicates with all departments regarding new resident move-in and internal resident move schedules ensuring any issues or concerns that may impact the new resident are brought to the appropriate department supervisor.
  • Serves as a liaison keeping appropriate staff aware of any resident and family concerns, changes, or potential issues that may arise throughout resident’s tenure at community.
  • Conducts resident care plan discussions involving resident and/or family as requested and/or needs change.
  • Develops and maintains an internal and external service resource directory, continually educating resident and family regarding available services, providing options and solutions to changing needs, getting other departments involved as needed to ensure satisfaction. (Examples include; Outpatient services, Home health services, financial assistance, eldercare law, grocery delivery, moving services, housekeeping, transportation or any other appropriate service as identified).
  • Regularly represents the organization in daily operations and at resident meetings/functions as a facility management resource to residents and families.
  • Assist with building and managing volunteer efforts.
  • Participate in community related events.
  • Provides backup to Social Services as needed
