Requirements

Must be at least eighteen (18) years or older, per Title 22 requirements
Experience working with older adults or other vulnerable populations.
Compassionate, patient, and enthusiastic personality with strong communication and interpersonal skills.

Responsibilities

Serve as the primary liaison between residents, families, and community teams to ensure a high-quality living experience.
Support residents through the transition into assisted living and ongoing adjustment to the aging process.
Identify and address resident needs by coordinating care services, documenting interactions, and advocating on their behalf.
Facilitate resident engagement through support groups, council meetings, and opportunities for community involvement.
Monitor and report resident concerns, including depression or isolation, and collaborate with the Care Management team to ensure appropriate interventions.
Support safety protocols, emergency plans, and elopement prevention practices.
Foster a warm, collaborative atmosphere with residents, families, and team members.
