POSITION PURPOSE: Clatsop Care Retirement Village's Resident Health Services Care Coordinator is employed and is directly responsible to the Administrator for ethical, professional, and adequate delivery of health services to residents. He/she will also facilitate and coordinate all health services employee educational requirements in accordance with State and Federal licensure for Assisted Living. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following: Makes every effort in good faith to support the success of every employee in the Clatsop Care Center Health District (including but not limited to peers, subordinates, supervisors and district administration). Collaborates with the Health Services Director (RN) shall participate in the development and implementation of any facility policies that affect the delivery of health care services. Ensures that all medications and treatments are given in accordance with Physician Orders and coordinates the medication delivery system. Solicits information from RN as necessary. Facilitates a quality assurance program for resident health care services along with Health Services Director (RN). Participates with the facility administrator and other department directors in development and maintenance of practices and procedures that promote infection control, fire safety, quality assurance and hazard reduction. Enters resident specific physician's orders into the computer as needed, prints, and maintains Medication Administration Records. Is available by telephone for staff consultation regarding scheduling - returns phone calls in timely manner. Assists RN with scheduling and training of Team Leaders. Willing and able report to work in the event of a call in and return calls in a timely manner. Assists RN with Incident Reports Completion. Prepares monthly vitals and weights to ensure they are completed at month end. Completes the Health Service component of orientation. Works with new caregivers and team leaders to ensure they have a successful transition to working on their own, including shadowing on multiple shifts. Prepares tasks for admissions and follows up to ensure tasks are complete. RN to ensure all necessary documents are received prior to move-in. Acquires therapy orders as directed by RN and Administrator. Participates in those committee meetings as assigned by the Administrator. Other duties as assigned by and in cooperation with the Administrator. All CCHD employees are mandatory reporters of abuse.
