About The Position

Country Village is seeking a compassionate, organized, and dependable Resident Care Coordinator to oversee the delivery of high-quality care and services for our residents. This role ensures compliance with facility policies, procedures, and professional standards while supporting staff and fostering a welcoming, home-like environment for residents. The Resident Care Coordinator works closely with the Administrator and care team to supervise Resident Aides, monitor resident wellbeing, maintain documentation, and ensure residents receive respectful, individualized care that promotes the highest possible quality of life.

Requirements

  • High School Diploma or equivalent required
  • Experience providing direct care to elderly residents required
  • Supervisory experience required
  • Weekend availability (Saturdays and Sundays) required
  • The role requires frequent physical activity, including bending, squatting, twisting, pushing, pulling, grasping, and lifting up to 50 pounds.
  • Must complete a physical examination and tuberculin (TB) test within required timeframes.
  • Must pass a criminal background check and fingerprint clearance through the Department of Social Services (DSS).
  • Must maintain a valid First Aid certification at all times.

Responsibilities

  • Support the Administrator in the daily operations of resident care services.
  • Ensure compliance with all applicable federal, state, and local regulations.
  • Supervise Resident Aides and oversee the quality of care and services provided.
  • Maintain up-to-date knowledge of resident status, census, and staffing needs.
  • Serve as a role model and leader for staff while maintaining effective communication with residents and families.
  • Ensure resident rights, dignity, and individuality are respected at all times.
  • Communicate with families and responsible parties as needed and promptly report complaints, allegations, or grievances.
  • Schedule and oversee bathing and activities of daily living to ensure resident needs are met.
  • Monitor Resident Aide documentation for accuracy and completeness.
  • Ensure monthly weights and blood pressures are completed and recorded; follow up on abnormal readings and report to the Administrator for physician notification.
  • Ensure adequate supplies are available for resident care (linens, personal care supplies, etc.).
  • Complete required medication assistance training and provide medication relief coverage if needed.
  • Develop and maintain monthly staff schedules and daily staffing sheets.
  • Process time-off requests and monitor attendance.
  • Review daily reports and charting to identify resident care needs requiring follow-up.
  • Monitor documentation, including 24-hour reports and 72-hour charting.
  • Ensure the cleanliness of laundry rooms, shower rooms, and linen storage areas.
  • Prepare and regularly review resident care records.
  • Review medication sheets monthly for accuracy and updates.
  • Attend shift change reports and review records, including bowel movement tracking and meal checks.
  • Ensure residents are accounted for at meals and that nutrition concerns are addressed promptly.
  • Conduct regular rounds to monitor resident grooming, hygiene, safety, and environmental conditions.
  • Perform room checks and monitor infection control practices.
  • Review resident service plans and ensure the care provided aligns with them.
  • Orient and train new care staff and support ongoing education and in-service training.
  • Perform other duties as assigned by the Administrator.
