About The Position

We are seeking a compassionate and organized Resident Family Liaison to support residents and their families during their transition into our skilled nursing facility. This role serves as a primary point of contact during the first two to three weeks following admission, offering guidance, reassurance, and clear communication between families and care teams. Responsibilities include welcoming new residents within 16 hours of admission, providing orientation to rooms and services, distributing welcome materials, addressing questions and concerns, facilitating communication, gathering necessary documentation (insurance, advance directives, vaccination records), and documenting interactions in the resident EMR. The ideal candidate has excellent interpersonal and organizational skills, proficiency in Microsoft Office, the ability to learn healthcare software systems, and prior experience in a skilled nursing or assisted living setting.
