The Case Manager provides high-quality, client-centered case management services to individuals enrolled in the RENEWPath Program at Janus of Santa Cruz. This position delivers comprehensive case management interventions including assessment, goal setting, care coordination, referral processing, and crisis intervention. Using evidence-based practices such as Motivational Interviewing, Strengths-Based Model , and ASAM , the Case Manager works collaboratively with the Program Manager and interdisciplinary team members to support client engagement and treatment outcomes. The Case Manager also processes referrals and assists with client care at the Sobering Center as needed. This role is ideal for an individual with a bachelor’s degree in a behavioral health-related field and/or equivalent experience providing case management or care coordination. The Case Manager must demonstrate flexibility, strong organizational skills, empathy, cultural humility, and the ability to thrive in a collaborative team environment.
