The Renal Care Coordinator, Outpatient is a non-licensed team member responsible for coordinating non-clinical tasks to support the delivery of case for high risk members with Chronic Kidney Disease (CKD) and End-Stage Renal Disease (ESRD). Responsibilities include, but are not limited to, assisting in care coordination, supporting member education activities, maintenance of member records and track progress, assume liaison role in the facilitation of dialysis appointments and transportation services, assist in monitoring member progress and ensure follow-up on referrals, and identify and address barriers to care such as transportation or non-adherence to treatments. The goal of the Renal Care Coordinator is to provide support and assistance in the overall care management of CKD and ESRD members on an outpatient basis to ensure the delivery of appropriate linkage of care, prevention of avoidable in-patient admissions and re-admissions, and provision of longitudinal care, including medical, nutritional, environmental, and psycho-social needs.
