CareHarmony partners with providers to deliver value-based care management initiatives for patients. Their clinicians are dedicated to providing high-quality care, assisting patients and caregivers in navigating the healthcare system, and helping patients manage chronic illnesses to improve their quality of life. Under the guidance of the Lead Triage RN, the triage nurse will be responsible for delivering exceptional patient care during after-business-hours calls. This includes telephonic triage for complex patients, addressing psychosocial needs, and supporting business initiatives.
