This role offers a unique opportunity for an experienced nurse practitioner (APRN) to provide exceptional preventive cardiovascular care to members with hypertension and atherosclerotic cardiovascular disease risk. You will work in a groundbreaking virtual specialty clinic aimed at improving access to care for vulnerable seniors. The clinic focuses on optimizing hypertension management and reducing associated risks, utilizing an advanced electronic health information platform for virtual consultations. Collaboration with an interdisciplinary team will be key as you contribute to tailored, specialized care for patients with chronic conditions.

Requirements

• APRN with 5+ years of outpatient clinical practice experience managing hypertension and related conditions.
• Minimum of 2 years focused on primary care or subspecialties emphasizing hypertension and lipid management.
• Proficiency with telehealth technology and electronic health records (EHR).
• Multi-state licensure required, with a willingness to maintain additional licenses.

Nice To Haves

• Virtual care experience preferred, with a passion for providing care directly to patients.
• Understanding of managed care, including assessing STARS/HEDIS measures and identifying care gaps.
• Bilingual in English and Spanish preferred.

Responsibilities

• Conduct focused assessments of patients with hypertension through virtual consultations, including ordering diagnostics and developing treatment plans.
• Formulate accurate diagnoses and individualize treatment plans, including medication management and lifestyle modifications.
• Initiate and manage statin therapy to mitigate cardiovascular disease risks.
• Collaborate with an interdisciplinary care team to coordinate holistic, patient-centered care.
• Serve as a clinical advisor for specialty clinic staff during business hours.
• Participate in panel review discussions to provide medical management advice.
• Utilize and improve the electronic health information system for virtual visits.
• Maintain accurate patient medical records, ensuring compliance with legal and ethical guidelines.
• Participate in quality improvement initiatives and professional development.
