This unique opportunity is designed for an experienced nurse practitioner with a compassionate, entrepreneurial approach. You will deliver exceptional preventive cardiovascular care to members with hypertension and cardiovascular disease risk. Working within a next generation virtual specialty clinic, you will expand access to care for America's most vulnerable seniors. Your role will involve optimizing hypertension management through virtual visits while collaborating with an interprofessional team to ensure high-quality, patient-centered care.

Requirements

  • 5+ years as an APRN in outpatient practice, focusing on hypertension and cardiovascular disease.
  • 2+ years of concentrated experience in primary care or relevant subspecialties.
  • Proficient in telehealth technology and electronic health records (EHR).
  • Experience in virtual care and a desire to continue practicing clinical nursing through virtual visits.
  • Understanding of managed care processes, including STARS/HEDIS measures.
  • Bilingual proficiency in English and Spanish is preferred.
  • Multi-state licensure with a willingness to obtain necessary licenses.

Responsibilities

  • Conduct thorough assessments of patients through virtual consultations, including ordering diagnostics and developing treatment plans.
  • Formulate accurate diagnoses and manage individualized treatment strategies for patients.
  • Initiate and manage treatment protocols for cardiovascular disease prevention, emphasizing lifestyle modifications.
  • Collaborate with an interdisciplinary care team to coordinate holistic care and ensure continuity.
  • Provide clinical escalation support for specialty clinic staff and participate in panel review discussions.
  • Utilize electronic health information systems to ensure effective patient management.
  • Maintain up-to-date patient medical records and compliance with ethical guidelines.
  • Engage in quality improvement initiatives to enhance care delivery.
