In this role, you will be responsible for managing a designated panel of members with varying health statuses and clinical needs. You will collaborate with a multi-disciplinary team and engage in outreach efforts to develop care plans, remove barriers to care, and enhance health outcomes for your members. Requirements

7 years of experience in clinical, case management, or disease/condition management.
High School Diploma/GED required; Bachelor's Degree in Nursing preferred.
Current RN licensure (State of PA or multi-state licensure) is required.
Preferred certification in Case Management.
Advanced training in cognitive behavioral therapy (CBT) or related fields is a plus.
Excellent written and verbal communication skills.

Responsibilities

Maintain oversight over a specified panel of members by assessing health management needs and identifying appropriate clinical interventions.
Create comprehensive care plans that address membersâ needs and promote health outcomes.
Monitor and ensure achievement of patient goal achievements and other applicable outcomes.
Document all activities while complying with business processes, regulatory requirements, and accreditation standards.
Stay current with applicable regulations and standards of practice for case management.
Perform additional duties as assigned.
