About The Position
The Remote Case Manager will be an integral part of a multi-disciplinary team within a home and community brain injury program. This role focuses on providing comprehensive case management services to vulnerable adults, ensuring they receive the necessary resources and support to enhance their quality of life. The Case Manager will conduct assessments, develop personalized care plans, and facilitate communication among healthcare providers, participants, and their families to promote self-sufficiency and optimal care outcomes.
