About The Position
Cecilia Holistic & Wellness Center is a community-rooted organization committed to bridging the gap in care by providing wraparound support services that promote wellness, stability, and self-sufficiency. We serve individuals and families with compassion, respect, and a holistic approach to care. We are looking for a dedicated, organized, and tech-savvy Care Manager to join our remote team. This position requires a deep commitment to community service and holistic wellness. While the role is fully remote, candidates must reside in the servicing county and be available for light local travel and driving for client visits, assessments, or community events.
Responsibilities
- Conduct virtual or in-person assessments to identify client needs and goals
- Develop individualized care plans and provide ongoing case management
- Coordinate services across housing, healthcare, employment, and wellness resources
