Remote Admissions Coordinator – Long-Term Care & Rehabilitation (RN or LPN) This position has local travel / field visits Location: Remote – Must reside in the Phoenix, AZ area with occasional local travel to skilled nursing facilities Employment Type: Full-Time Salary: $80,000–$95,000 annually, based on licensure and experience Position Overview A leading managed care organization serving Arizona Medicaid members is seeking an experienced and detail-oriented nurse to support long-term care and rehabilitation admissions throughout the Phoenix metropolitan area. This fully remote position focuses on coordinating placements into skilled nursing facilities and extended care settings while ensuring timely, appropriate transitions of care. This role is ideal for an RN or LPN with experience in long-term care, home health, hospital discharge planning, or post-acute admissions who understands Arizona Medicaid and level-of-care criteria.

Requirements

  • Active, unrestricted Arizona RN or LPN license required
  • Minimum 2–3 years of experience in long-term care, home health, rehabilitation, or hospital admissions
  • Working knowledge of AHCCCS and Arizona long-term care guidelines, including ALTCS
  • Strong communication, organization, and documentation skills
  • Ability to work independently in a remote environment
  • Proficiency with electronic medical records

Nice To Haves

  • Managed care or health plan experience
  • Utilization management or case management experience
  • Experience with hospital discharge planning or SNF placement
  • Bilingual English and Spanish a plus

Responsibilities

  • Coordinate admissions into long-term care, skilled nursing facilities, and rehabilitation programs
  • Review clinical documentation to determine appropriate level of care
  • Facilitate transitions from hospitals to SNFs or long-term care settings
  • Serve as liaison between the health plan, facility admissions teams, and discharge planners
  • Manage prior authorizations and eligibility verification within scope of licensure
  • Collaborate with utilization management and case management teams
  • Educate members and families regarding benefits and care transitions
  • Maintain accurate documentation in EMR and care coordination systems
  • Ensure compliance with AHCCCS and Arizona regulatory requirements
