About The Position

Description Duties and Responsibilities Is available to work, at a minimum, for each of the agency’s programs in the employee’s home county. Completes at least one relief shift per calendar quarter. Reviews and signs-off on the job description(s) for which coverage is requested. Performs duties of the position(s) as assigned. Mains required credentials for the job description(s) for which coverage is requested. Is responsible for reviewing agency and program policies and procedures. Signs-off annually on required documentation (e.g. code of conduct, professionalism policy) Participates in required agency trainings. Is mindful of and integrates Trauma-Informed Care and its five principles in all duties and interactions with fellow staff, residents, and external partners. Maintains compliance with all required rules and regulations including Federal Confidentiality (42 CFR, Part 2), HIPAA, Medicaid and/or HUD, and any others.

Requirements

  • One year’s experience practicing as a Licensed Practical Nurse.
  • Requires an unrestricted New York State License or Certification: Licensed Practical Nurse, CPR and First Aid Certified.
  • Must possess a New York State driver’s license with an acceptable Motor Vehicle Record.
