About The Position

Partners In Care (PIC) is an independent 501(c)(3) nonprofit organization and the largest, oldest provider of home health, palliative care, and hospice services in Central Oregon. Serving Deschutes, Crook, and Jefferson counties, our dedicated team of over 200 employees and 200 volunteers delivers compassionate, patient-centered care across a 10,000-square-mile area. Additionally, Hospice House in Bend, Oregon, is a specialty hospital for inpatient hospice care, creating an oasis of tranquility and comfort for patients and their families. Status: Relief/Casual (hourly, non-exempt) - less than 23 hours per week. Relief/Casual employees must offer a minimum of four shifts per month, including one night shift and one weekend shift. Hours: Day Shift: 7:00 AM-7:00 PM and Night Shift: 7:00 PM-7:00 AM. Additional shifts, including day, night, and weekend shifts, may be available. Available shifts are based on departmental needs and are subject to change; a fixed schedule is unavailable for this role. Location: Hospice House in Bend, Oregon. Summary/Objective: The Hospice House RN will identify patient/family/caregiver needs and provide nursing and supportive care in accordance with the plan of care, and policies and procedures.

Requirements

  • Valid Oregon RN License.
  • Ability to pass a background test, drug screen, and TB test.
  • Graduate of an accredited nursing school.
  • One to two years of recent medical care experience in an institutional setting.
  • Valid CPR Certification.
  • Strong communication and interpersonal skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Compassion, empathy, and dedication to patient care.

Nice To Haves

  • Ideal candidates will possess 1-2 years of home health or hospice experience (preferred).

Responsibilities

  • Apply nursing principles and techniques in the treatment of patients.
  • Provide nursing care, emotional support, and education to the patient/family/ caregiver.
  • Maintain strict confidentiality of patient, family and caregiver information.
  • Is responsible for the implementation and continued development of the patient care plan and physician's plan of treatment and is responsible for the case management of the patient.
  • Supervision of all Certified Nursing Assistants and Volunteers.
  • Follow the guidelines for universal precautions when providing patient care and demonstrate proper techniques in application of infection control standards.
  • Maintain records of services performed and of apparent condition of patient, using documentation guidelines and standards set forth by the facility.
  • Communicate with physicians and others as needed to coordinate and ensure high quality care.
  • Communicate on a regular basis with all staff / volunteers regarding patient status.
  • Demonstrate a working knowledge of the dying process and recognizes the importance of and maintains professional boundaries.
  • Provide for a safe environment for patients, guests, visitors, and employees and adhere to all policies, procedures, and regulations regarding patient care, conduct, safety, fire, security, and risk management.
  • Participate in weeknight and weekend on-call duties as defined by the on-call policy.
  • Complies with all applicable laws, regulations, policies and procedures.
  • Adheres to the PIC mission and values.
  • Supports the organization.
