SUMMARY DESCRIPTION OF WORK Professionals at this level have excellent communication skills with both client and community institutions. Specific project, program or office responsibilities may occur at this level. Decision-making, problem solving, and highly developed interpersonal skills are critical. This position functions as a fully integrated Team Member of the Assertive Community Treatment Team, which is a multi-disciplinary team, that assists individuals with a serious mental illness to minimize symptoms and the resulting impairment in daily functioning. Requirements

  • Bachelor’s Degree in Human Service field.
  • Three years in case management, vocational or skill development experience.
  • Valid driver’s license, acceptable driving record.

Responsibilities

  • Client assessment of individual skills/interests and formulation of a plan for employment/education goals.
  • Assist clients in developing skills in the areas of daily living, social and interpersonal relationships, and occupational capabilities.
  • Provide support and consultation to team members and community partners.
  • Maintain all contact documentation and comply with contract conditions.
  • Provide problem solving and crisis intervention as needed.
  • Coordinate services with other community agencies and resources.
  • Provide service by helping clients manage their finances.
  • Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the agency.
  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
  • Is dependable and punctual regarding scheduling and attendance.
  • Abides by all specific program and Agency procedures, policies, and requirements.
  • Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
  • Performs other program related duties as assigned.
