About The Position

As a Rehab Director with Legacy Healthcare Services, you will be the primary Legacy representative at the facility, promoting Legacy policies and clinical programs to help aging adults live safe, healthy lives. This role focuses on building relationships with patients, learning new ways to deliver care, and being part of a team dedicated to helping senior patients. Legacy Healthcare Services is a not-for-profit organization that provides personalized therapeutic care for residents within senior living communities, aiming to improve their quality of life. The organization was founded by and continues to be led by therapists, serving nearly 500 senior living communities across the U.S. They are recognized as leaders in innovation and personalized care, making them a preferred provider for senior adults.

Requirements

  • Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Speech Language Pathologies (SLP).
  • Three to five years’ experience in long-term care (LTC) or senior living.
  • Minimum of a degree from accredited college or university or equivalent experience (i.e., degree in PT, PTA, OT, COTA, SLP).
  • Licensed and registered (PT, PTA, OT, COTA, SLP) in state where assigned.

Nice To Haves

  • Some prior supervisory experience preferred but not required.

Responsibilities

  • Serve as the primary Legacy representative at the facility, promoting Legacy policies, clinical programs and supporting our mission to help aging adults live safe, healthy lives full of strength, movement, and joy.
  • Create rapport and positive working relationships with key facility personnel and fellow Legacy employees that allows for discovery of others’ needs.
  • Oversee internal marketing efforts to assist with relationship building with staff and residents while keeping community leaders apprised of Legacy’s programs and other offerings.
  • Manage site productivity and makes necessary changes to achieve budget.
  • Maintain a caseload that assists the rehab department in meeting standards.
  • Assess patient needs and develop clinical programs appropriate for the facility census.
  • Direct and train others in clinical service delivery across disciplines.
  • Daily coordination of rehab department with facility staff to plan and implement therapy programming.
  • Participate in clinical outcomes reporting and shares with the customer.
  • Manage staff schedules to meet the caseload requirements and utilizes the scheduling tools for patient schedules.
  • Manage CliniFax and all requirements of physician signatures on the therapy plan of cares.
  • Create and maintain team relationships and open communication with and between rehab staff, facility staff, and Legacy corporate personnel.
  • Assist with orientation/training for staff with guidance and assistance from ARD/ MSM to meet company, state and federal standards.
