About The Position

Bridge Housing is intended to address the immediate shelter, and treatment needs of people who have serious behavioral health conditions and are experiencing homelessness. The site management and support services team are responsible in maintaining a 80-bed transitional recovery and community re-entry housing facility for those suffering from mental illness and/or substance use disorder who are experiencing homelessness. The target population often suffers from complex and co-occurring medical conditions and other characteristics that can challenge their ability to navigate mental health, physical health, and substance use treatment in the Santa Rosa community. Services include 24/7 medical and clinical staffing with a department-assigned Housing Coordinator who will provide “gatekeeping” to assure appropriateness of all referrals. Referrals must be clients with a mental illness and/or co-occurring substance use disorder and are either precariously housed or are unsheltered. Position Summary Under the supervision of the Medical Director & Division Director of Justice Services, the Registered Nurse provide direct and on-call support and nursing administrative oversight for members of the team Coordinates and conducts appropriate trainings for program staff; facilitates program development and growth RN is responsible for the overall management of patients, including the provision of direct care with a designated visit productivity standard This position plans, organizes, and directs all resident care services in the assigned caseload in accordance with current standards and regulations, ensuring the optimal degree of quality care is maintained Responsible for coordinating the interdisciplinary team to implement the established plan of resident care and ensuring effective management of visit utilization and control of expenses Ability to communicate with residents and significant others in an effective, mature, caring manner. Facilitate care along a continuum through effective resource coordination. Monitor the health of residents: This includes tracking vital signs, managing medication, and observing changes in health conditions Check vital signs and look for signs that health is deteriorating or improving. Perform basic nursing functions such as changing bandages and wound dressings. Nurses are expected to liaise with doctors, therapists, and other healthcare professionals when applicable to ensure a comprehensive care plan for each resident Work with and manage Quality Assurance staff to provide guidance in proper charting/documentation practices to meet all county standards and funding requirements. Exhibits knowledge of Pathophysiology and accepted treatment protocols for common resident care diagnoses Participate in weekly staff meetings and participate in agency-level meetings monthly and as needed and collaborate closely with community partners for referrals, linkage networks, etc.: Sonoma County departments, community-based social service providers, etc. Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements Responsible for maintaining all required licensure and certifications

Requirements

  • Graduate of an accredited School of Nursing
  • Pass the National Council Licensure Examination (NCLEX-RN)
  • Bachelor’s degree in nursing (preferred) associate’s degree in nursing, or a diploma from an approved nursing program required. Registered nurses must be licensed
  • Excellent verbal and written communication skills
  • Strong collaboration and coordination abilities
  • Proficiency in care management practices
  • DL-Valid Driver’s License Class C
  • 2 years recent relevant experience
