About The Position

Utilizes nursing assessment skills effectively. Coordinates and completes patient assessment. Consistently assesses pain, using the appropriate scale, intervention, and reassessment tool. Monitors effects of medications and reports adverse effects. Assesses potential discharge needs. Assesses barriers to learning and functional status of patient. Age Specific Competencies: Encourages patients to report pain and to communicate concerns regarding pain management. Anticipate patient may focus on time constraints and concerns impacting life style, family, work, and/or income loss due to treatment. Demonstrates competent clinical skills. Demonstrates knowledge and skill of equipment and procedures. Demonstrates knowledge of medication administration and safety according to hospital policy. Identifies and reports all risk management issues. Demonstrates competency for chemotherapy/infusion administration. Demonstrates knowledge/ adherence to Blood Administration. Age Specific Competencies: Aware of special considerations based on distinguishing characteristics of age groups. (ex. elder adults may have unpredictable absorption medications and require close monitoring) Mains complete and accurate documentation. Completes required documentation in accordance with hospital policy. Patient’ s plan of care is complete and documented in a timely manner. Patient and family response to treatment plan and nursing interventions is documented. Documents changes in patient condition. Verifies MD orders and daily charges. Documents telephone orders in accordance with policies. Assess emotional distress daily and reports 4 or higher scores to SW. Completes PHQ questionnaire for depression screening and refer to social worker for score of 3 or higher. Age Specific Competencies: Aware of and follows the organizations policy regarding the reporting of suspected elder abuse and domestic violence and depression . Assumes responsibility for directing patient care. Delegates tasks to the appropriate team member. Carries out physician orders in an accurate manner. Coordinate tests/procedures. Prioritize based on changes in patient s condition. Coordinates and implements patient/family teaching. Collaborates with other disciplines such as pharmacy, social work, dietary. Age Specific Competencies: Identifies psychosocial issues. Makes timely referrals to social worker. . Demonstrates professional practice responsibilities. Completes annual clinical competencies as required on unit. Demonstrates ability to prioritize care, and problem solve to meet the changing needs of patient. Demonstrates knowledge of policies and procedures. Functions as a resource/mentor to staff members. Identifies knowledge/skill gaps and takes action in obtaining education and/or training. Attends meetings and keeps current with changes in profession and unit function/activities. Participates in hospital and/or unit-based committees. Job Function: 06. Patient Satisfaction. Relays information regarding plan of care, patient progress, scheduling of tests/procedures to patient and family. Includes patient and surrogate in care planning. Act as conduit of information for patient and family. Professional Practice Functions. Demonstrates willingness to float to other departments.
