REGISTERED NURSE - Adult Day Program

Our Day Habilitation Program is seeking an RN to provide instruction and support, as well as oversee all health-care services provided to the program participants. This position will coordinate medical services with each program participants primary physician or medical clinic. This position will participate in ongoing interdisciplinary evaluations and review of each program participants needs and status.
Requirements
- Valid RN license in Massachusetts
- One year of experience in a community health setting
- Skill in communicating and working both independently and within a team.
- Ability to complete and disseminate all required forms and documents
- A qualifying criminal background check and motor vehicle record.
Responsibilities
- Provide direct care and training in relevant areas.
- Coordinate medical services with each program participants primary physician or medical clinic.
- Oversee all health-care services provided to the program participants.
- Participate in ongoing interdisciplinary evaluation and review of each program participants needs and status.
- Obtain reports and approval of treatment plans from primary physician.
- Document in each program participants record any recommendation resulting from the periodic reviews.
- Advise the Program Director and the day-habilitation staff of any medical problems that may hinder a participants progress or that may be aggravated by activities at the program.
