We specialize in providing Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Language Pathology, Medical Social Worker, Home Health Aide, Chaplain, Dietician, and Hospice Volunteer Services for patients in their home setting throughout Josephine, Jackson and Douglas counties. Responsibilities

  • Providing skilled, one-on-one nursing care in patients’ homes
  • Educating and supporting patients and caregivers
  • Managing medications, symptoms, and complex clinical needs
  • Collaborating closely with physicians and care teams
  • Supervising LPNs and CNAs as appropriate
