Requirements

Must have the desire to work in Memory Care, do case management, and have a dedicated patient load.
Current Minnesota RN License in good standing required
Must be 21 years or older

Nice To Haves

Experience in Memory Care and or Assisted Living preferred
Must desire the opportunity to work with a team of nurses that collaborate, motivate, and support one another.

Responsibilities

Ensuring the health and safety of all residents through assessments and reassessment
Development and implementation of appropriate service plans and plans of care
Monitoring resident services
Supervision of staff
On-going staff training
Complete an initial, face-to-face comprehensive assessment of prospective/new residents and develop a proposed service plan
Conduct or delegate the completion of assessments as resident conditions change
Staff education
Case management
Have a dedicated patient load
