The Registered Nurse provides both direct and indirect clinical support to individuals throughout Achieva Support. This role is responsible for providing medical guidance to staff, performing nursing assessments, and coordinating care with external healthcare professionals. You will serve as a key clinical liaison to ensure medical and behavioral health needs are met with excellence and compassion.

Requirements

  • Graduate of an accredited nursing program
  • Valid Pennsylvania RN license required
  • Minimum of two (2) years of experience supporting individuals with intellectual disabilities and medically complex needs.
  • Proficiency in computer applications
  • Ability to become proficient in the Therap system.
  • Strong organizational, communication, and interpersonal skills
  • Ability to provide constructive feedback and training.
  • Must have a reliable vehicle and a valid driver's license for local travel
  • Ability to work flexible hours including evenings and weekends.
  • Must be able to pass required background checks, including Act 33, Act 34, and FBI clearances.

Responsibilities

  • Provide nursing assessments and consultation for medical and behavioral health situations to ensure all health needs are addressed.
  • Deliver instruction to employees on medical care requirements, nutrition, hydration, skin integrity, and infection prevention.
  • Manage hospitalizations, admissions, and discharges; maintain contact with hospital or skilled nursing facility (SNF) staff to coordinate safe transitions home.
  • Act as a liaison with pharmacies; prepare monthly Medication Administration Records (MARs) and process orders within the Therap system.
  • Communicate practitioner recommendations to staff and collaborate with program leadership on clinical and mental health recommendations.
  • Respond to medical emergencies or crisis situations via phone or on-site, ensuring proper notification of families and professionals.
  • Participate in the Health Risk Screening Tool (HRST) process as a Clinical Reviewer and contribute to the development of clinical policies.
  • Participate in program licensure inspections and track state health care initiatives for best practices in the IDD system.
  • Participate in an after-hours on-call rotation to provide clinical guidance to management and staff.
