Make a meaningful impact every day. Hillview Health Care Center is seeking a compassionate and skilled Registered Nurse to join our dedicated team at our Long-Term Care Campus. This professional nursing position plays a key role in resident care, team leadership, and upholding the highest standards of clinical excellence. Overview FLSA: Non-Exempt. This is a professional nursing position at the Long-Term Care Campuses of La Crosse County. Employees are selected by, and work under the general supervision of the Director of Nursing Services. Employees in this class are responsible for implementation, supervision and evaluation of resident care and management of the Licensed Practical Nurses, Nurse Technicians and Nursing Assistants on a particular shift or living area. Location: Hillview Health Care Center, La Crosse, WI Schedule: AM Shift | 6:30 AM to 3:00 PM | Every other weekend & holiday rotation Pay scale: $35.09 to $40.69 per hour + shift differentials Status: Irregular Part-Time (less than 40 hours per pay period)

Requirements

• Requires graduation from an accredited nursing program with current Registered Nurse license in the State of Wisconsin.
• Demonstrate leadership skills.
• Those persons driving must have and maintain a valid Wisconsin driver license and good driving record.
• CPR certification.
• Must maintain a valid Wisconsin nursing license.
• Must successfully pass a background check as required by the Wisconsin Caregiver Law.
• Ability to supervise nursing staff.
• Ability to cope with various nursing situations both mentally and physically.
• Ability to coordinate activities and determine work assignments of subordinate personnel and supervise their job performance.
• Ability to record observations with accuracy and clarity.
• Ability to communicate effectively and establish and maintain good working relationships with other staff members, other health care professionals and employees.
• Comprehensive knowledge of nursing theories and skills.
• Thorough knowledge of application of a nursing process, assessment, planning, implementing and evaluating resident care; developing and updating care plans.
• Basic skills in using computers to effectively enter documentation data in a computer.
• Considerable knowledge of residents needs in care of geriatric and/or mental health.

Responsibilities

• Provides care within the provisions of the Nurse Practice Act.
• Provides direct nursing care including administering medications and treatments.
• Supervises all nursing activities on assigned living area.
• Evaluates and determines resident needs and initiates and revises the resident care plan.
• Contributes to the multidisciplinary resident care planning.
• Provides for implementation of all medical orders; assists physician, as needed; collaborates with the Nurse Managers to prepare information for physician's rounds.
• Makes clinical assessments of resident condition and behaviors.
• Determines appropriate action based on these assessments.
• Makes accurate and complete nursing notes of these observations and actions in the clinical records.
• Documents accurately and completely all observations, interventions and procedures relating to resident care.
• Directly supervises the performance of nursing tasks assigned to Nurse Technicians.
• Determines and supervises work assignments to provide maximum utilization of Nursing Assistants and is responsible to see that assigned tasks are accomplished.
• Provides feedback to nursing assistants regarding job performance.
• Assists in the training and orientation of new staff.
• Ensures correct and appropriate diet for residents.
• Documents all observations and procedures relating to resident care accurately and completely.
• Prepares daily shift reports and keeps supervisory nurse informed.
• Employee is expected to report to work when scheduled, with minimum use of sick leave, to build up sick leave balance for income protection for unexpected illness.
• Works safely and follows safety practices and standards of the facility; responsible for reporting or correcting any existing or potential safety or accident hazard.
• In this public service position, employee is required to be courteous, cooperative and respectful at all times, with the public and clients.
• Also establishes and maintains a courteous, cooperative and respectful working relationship with other employees, supervisors and public officials.
• Maintains residents' rights and confidentiality of resident information; reports all suspected cases of resident abuse to the Administrator.
• Immediately reports all suspected cases of resident abuse, neglect or misappropriation of funds to the supervisor.
• Attends in-service education as provided or mandated.
• Supports all aspects of person centered care.
• Respects and acknowledges individuality and uniqueness of residents.
• Responsible to assist the MDS Coordinator to provide for the timely and accurate completion of the MDS (Minimum Data Set) for nursing home resident assessment and caring planning.
• Will also assist with the Restorative Nursing program.
• Demonstrates strong clinical nursing skills in assessment, planning, implementation and evaluation of nursing interventions for facility residents.
• Responsible for the accurate and timely completion of the MDS, as required by OBRA/Federal regulations and PPS.
• Coordinates and/or participates in care planning process, especially for residents with complex care needs.
• Demonstrates knowledge and clinical competence in area of resident care standards.
• Thorough understanding and working knowledge of the current MDS and RUGS systems.
• Thorough knowledge of application of the nursing process, assessment, planning, implementing and evaluating, completing accurate assessments and MDS entries that will result in correct resident assignment to the proper RUGS IV to ensure the appropriate payment to the facility.
• Ability to maximize RUGS scores through accurate MDS coding by capturing restorative participation, skilled nursing interventions and therapy.
