Responsibilities

Deliver person-centered nursing care in alignment with Mayo Clinic standards. Excellence in assessing, diagnosing, outcome identification, planning, implementing, and evaluating is the basis for all care delivered by the Registered Nurse.
Monitor and manage residents’ health needs across multiple care settings. You will be responsible for the delivery of direct resident care and supervising nursing care provided by the professional and ancillary nursing team across care settings including Individualized Health Services, Catered Living, and Enhanced Living.
Collaborate with interdisciplinary teams to ensure resident safety and well-being. As an RN at Charter House, you will assess and plan in collaboration with the resident, family and the interdisciplinary health care team, to meet the care, educational, and discharge needs.
Accurately document skilled interventions to reflect clinical judgement and resident needs.
