Growing together. You will be part of the primary care team collaborating with all interdisciplinary team members and will support multiple providers. The RN is a flexible team member who works alongside nurse practitioners (NPs) and physician assistants (PAs). This is a field-based position, visiting members primarily in nursing homes.

Requirements

  • Associate degree or higher in Nursing (RN)
  • Active and unrestricted license in Texas
  • 1+ years of clinical experience
  • Certified in Basic Life Support
  • Ability to complete physical requirements, including lifting a 30-pound bag and ability to perform physical assessment
  • Access to reliable transportation to travel in between facilities

Nice To Haves

  • BSN
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Saline Lock insertion experience
  • Understanding of Geriatrics and Chronic Illness
  • Proficient computer skills including the ability to document medical information with written and electronic medical records

Responsibilities

  • Collaborating with all members of the interdisciplinary care team around changes in condition and/or care coordination
  • Triage member needs to the appropriate care team member and level of care
  • Assist with transitions in care: outpatient and inpatient
  • Assist in assuring complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessment
