About The Position

Schedule: Shift: 9:00 AM - 5:00 PM Travel: 70% office-based; travel to patient homes (Manhattan, Brooklyn, Bronx, and Queens) for wound care consults as needed Caseload: 15-20 consults per day

Requirements

• Active New York State Registered Nurse License
• Wound Care Nurse Certification or Wound Ostomy Care Nurse Certification
• BCLS Certification
• Bachelor’s Degree in Nursing (required)
• Minimum 2 years in a Certified Home Health Agency or post-acute care setting
• 2 years of medical/surgical experience
• 2 years of public/community health experience
• Familiarity with performance improvement initiatives and relevant software applications
• Knowledge of EPIC H2O, Microsoft Office Suite, and PI applications
• Experience with wound care equipment, including Varisource/HDR and CT Simulator
• Strong verbal, written, organizational, and analytical skills
• Flu vaccine required (medical/religious exemptions accepted)
• COVID-19 vaccine not required

Responsibilities

• Assess and treat complex wounds, ostomies, pressure injuries, burns, and diabetic foot wounds.
• Collaborate with an interdisciplinary team to develop individualized, evidence-based care plans.
• Provide patient and family education on wound care, infection prevention, and treatment measures.
• Participate in the development and revision of wound and ostomy care policies and protocols.
• Train and evaluate field nurses on wound care techniques and documentation.
• Conduct quality improvement initiatives, case load reviews, and chart audits.
• Ensure compliance with state, federal, and agency policies and procedures.
