About The Position

"Navitas Healthcare, LLC" is seeking an experienced Wound Care Registered Nurse to provide specialized care in wound, ostomy, and continence management. This role involves conducting comprehensive patient assessments, developing individualized plans of care, delivering evidence-based treatments, and collaborating closely with the interdisciplinary healthcare team to ensure optimal patient outcomes.

Requirements

  • Minimum of two (2) years of Med/Surg nursing experience
  • Demonstrated experience in wound and colostomy care
  • Associate Degree in Nursing (ASN/ADN) from an accredited nursing program
  • Active New York State Registered Nurse (RN) License
  • Primary Source Verification through the NYS Department of Education (NYSED)
  • AHA Basic Life Support (BLS)
  • Current certification in one of the following: Certified Wound Ostomy Continence Nurse (CWOCN) – WOCNCB Certified Wound Ostomy Nurse (CWON) – WOCNCB Certified Wound Care Nurse (CWCN) – WOCNCB Certified Wound Specialist (CWS) – American Board of Wound Management (ABWM) Wound Care Certified (WCC) – National Alliance of Wound Care and Ostomy (NAWCO)

Responsibilities

  • Conduct health assessment interviews and focused physical assessments related to wound, ostomy, and continence care
  • Develop, implement, revise, and evaluate individualized plans of care
  • Recommend appropriate medications, treatments, supplies, and equipment
  • Assess, document, and evaluate patient responses to wound, ostomy, continence, fistula, and complex wound treatments
  • Collaborate with clinical staff and interdisciplinary teams; initiate referrals as appropriate
  • Educate and counsel patients and families on prevention of complications and self-care management
  • Develop, deliver, and evaluate staff education programs, including orientation, in-services, and continuing education
  • Perform routine assessments of patients at risk for pressure injuries and initiate preventative interventions
  • Communicate and escalate identified challenges, including ethical concerns and conflict resolution issues
  • Collaborate with nursing leadership to recommend and implement wound care best practices
  • Participate in unit-based and hospital-wide quality improvement initiatives, including incidence and prevalence studies
  • Provide culturally competent education to patients, families, and staff
  • Maintain confidentiality and security of patient information in accordance with organizational policies
  • Maintain professional competency through ongoing clinical practice and education
  • Identify and coordinate community resources to support patient care
  • Manage assets related to rented equipment, including specialty beds and negative pressure wound therapy devices
