About The Position

Responsible for the assessment of patient to ensure the appropriateness of line selection based on patient history, clinical condition and planned therapy. Provides direct patient care and education to patient and caregivers/staff in the appropriate use and care of Vascular Access Devices/ PICC lines. Serves as a resource and provides leadership in the development, coordination, and monitoring of Vascular Access Devices and/or PICC line care, and system measurements to improve clinical outcomes. What you will do Coordinates and provides patient care/services in accordance with established protocols, policies and procedures to ensure appropriate assessment and placement of a Vascular Access Device and/or PICC line. Evaluates the effectiveness of patient care/services and makes adjustments/recommendations as needed to improve patient care and services. Plans, organizes and delivers care within clinical units/areas consistent within nursing division policies and procedures, demonstrates competencies, and with consideration given to risk management and infection control practices, and patient safety practices. Participates as a member of the multi-disciplinary team to develop, implement, and evaluate individual care plans based upon the assessment, diagnosis, and expected outcomes of patient. Facilitates the achievement of the defined standard of intravenous care for all patients throughout the facility. Assists in developing, implementing and interpreting standards and policies/procedures and best practice principles regarding vascular access Devices and PICC/central line placement. Oversees quality assurance programs. Evaluates the effectiveness of care/services and makes adjustments/recommendations for appropriate additions or deletions to assure quality and cost-effectiveness of products used for vascular access Devices and PICC/central line care as needed to improve patient care and services. Assists staff in maintaining competence in the areas vascular access Devices and PICC/central line care. Provides in-services to staff as needed. Provides consultation and assistance to staff in the management of patients with vascular access Devices and PICC/central line. Works with nursing unit staff and Case Management to identify patients who may need PICC line placement. Assesses patients for Vascular Access Device needed and assist in selection on the most appropriate device. Assists staff by monitoring PICC line insertion sites, declotting, performing dressing changes and discontinuing catheters with or without catheter tip cultures. Contact physicians for medical orders for PICC line placement. Obtains patient/family consent for PICC line placement. Provides assessment and placement of peripherally inserted central catheters using ultra-sound and micro-introducers. Assists in maintaining the PICC line and/or refer placement to Interventional Radiology PICC staff. Ensures chest x-ray is complete and placement documented on all newly place or reposition PICC lines to verify the position of the catheter tip. Provides peripheral line support to nursing for difficult to access patients. Monitors and follow-up on all PICC lines throughout facility. Facilitates the achievement of the defined standard of intravenous care for all patients throughout the facility. Assesses and identifies learning needs of staff, patients and families regarding the care and maintenance of Vascular Access Device and/or PICC line. Assists in the development, implementation, and evaluation of services and materials for patients, staff and other professionals. Engages in patient/family/staff teaching to achieve the desired patient outcomes. Participates as an active member in staff meetings and committees. Demonstrates effective work organization in stressful situations and as part of the patient care team. Plans and conducts classes, in-services, activities or individual sessions to meet identified learning needs; develops curriculum, teaching materials, lesson plans and evaluation tools. Maintains awareness of financial issues, cost containment, and resources for patients. Establishes and maintains effective working relationships with members of the health care team, patients, and families. Interacts and communicates with members of the health care team, patients, families and visitors in a customer responsive and courteous manner and in compliance with iRESPECT standards. Demonstrates a service philosophy in all interactions with patients, families, and all members of the health care team. Works collaboratively and serves as a resource person to medical, nursing and healthcare team to achieve the patient’s expected outcome; consultant to health care providers/organizations/agencies and those participating in care/teaching plan of inpatient or follow-up care of outpatients. Maintains professional credentials and education necessary to keep abreast and maintain a current knowledge of clinical/theoretical principles, concepts and techniques. Attends continuing education, staff development, and mandatory programs. Keeps current on evolving PICC line therapy literature, research and standards related to vascular access and IV therapy. Maintains proficiency in use of Ultrasound to assess vessels and vessel health. Participates on the Value Analysis Committee to evaluate new products vascular access devices or discuss product capabilities or deficiencies related. Provides instruction to nursing and other support/ancillary personnel to complete the initial and ongoing assessments of patients and to provide clinical assistance as needed. Participates in staff training and performance improvement activities and utilizes the results of the quality of care activities to initiate changes in practices. Serves as a resource and mentors nursing staff in development of vascular access abilities and IV therapy skills. Provides orientation and continuing education for nursing staff and other relevant clinicians regarding central venous catheter standards, access and care. Collects and reports PICC line data: percentage of PICCs placed successfully on first attempt as confirmed by chest x-rays report; vascular device line infection rate and catheter migration. Assists in the orientation and oversee the supervision of new staff members and students when they provide patient care, treatment, and services as part of their training. Maintains a clean, orderly, safe environment for patients and personnel. Adheres to established policies, compliance/quality improvement, and safety, environment and infection control standards. Prepares and maintains required records and statistics for operational, administrative and regulatory purposes.

Requirements

  • Graduate of an approved nursing school/program Required
  • Qualified by education, training or experience to work with patient population as specialty assignment dictates Required
  • RN - Registered Nurse - State Licensure and/or Compact State Licensure licensure to work in the state of TN Upon Hire Required
  • CRNI - Certified Registered Nurse Infusion within specified time frame Upon Hire Required or VABC - Vascular Access Board Certified within specified time frame Upon Hire Required
  • Minimum 2 years experience recent hospital experience in critical care or emergency nursing Required

Nice To Haves
