About The Position

Under the general supervision of the Associate Medical Director and Triage Supervisor, the Triage Nurse is responsible for providing clinical advice and related nursing duties, participating in quality assurance, peer review and multidisciplinary team case management. The Triage nurse is the liaison between the central call center, patients and provider teams. Works in collaboration with clinical teams, specifically Team RNs, to provide support in the delivery of safe and timely care for health-related problems. The Triage RN demonstrates ability to make decisions based on a solid foundation of critical thinking skills. The Registered Nurse – Triage reports directly to the Medical Director. This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

  • Experience with electronic health records, specifically NextGen, strongly preferred.
  • Availability to work a minimum of two full days per week, and occasionally to sub extra days for absent Team RN’s.
  • Minimum two years clinical experience (LVN experience considered), especially in the area of chronic care management in a community healthcare setting.
  • Bachelor’s Degree in Nursing.
  • Current and valid California Registered Nurse license and BLS.
  • Proficient in Microsoft office suite.

  • Previous experience with homelessness, substance abuse, and psychiatric disorders preferred.
  • Experience as part of a team-based care model preferred.

Responsibilities

  • Provides appropriate nursing care, including assessment, triage, and referral, while emphasizing patient comfort and well-being.
  • Coordinates medical care for primary care patients, initiates and follow up diagnostic tests, medical reports, referrals, and emergency care.
  • Participates in quality assurance.
  • Participates in multi-disciplinary team case management.
  • Coordinates primary care clinic services with other providers and agencies.
  • Provides well-documented, accurate and sensitive telephone and in-person drop-in advice, and well as health education and self-care instruction.
  • Participates in community outreach and marketing activities by attending Health Fairs to promote the organization’s activities.
  • Gives presentations on clinic services to interested organizations or civic groups.
  • Triages phone calls and urgent walk-ins; schedules urgent appointments. Takes urgent warm hand offs from the call center or staff at EO.
  • Actively participates in pre-clinic daily huddle, identifies high needs patients for the clinic session, conducts preliminary patient assessments, and anticipates clinician needs.
  • Calls new patients and hospital follow-ups the day prior to identify needs and prep for the hospital visit.
  • Provides follow-up/case management that may be needed pre/post-clinic visit.
  • Triages phone calls and urgent walk-ins; schedules urgent appointments.
  • Provides chronic care management education and skills teaching to patients.
  • Uses clinician-approved standing orders to order labs and preventive screenings, and to refill routine medications.
  • Actively participates in conducting preliminary patient assessments and anticipates clinician needs.
  • Provides follow-up/case management that may be needed pre/post-clinic visit.
  • Provides chronic care management education and skills teaching to patients.
  • Uses clinician-approved standing orders to order labs and preventive screenings, and to refill routine medications.
  • Functions as a Team RN (member of multidisciplinary primary care team), as needed.
  • Works closely with the Triage Assistant to assess medication-related issues and DME orders and is the back up in the absence of the Triage Assistant.
  • May be asked to dispense medications from the Patient Assistance Program for uninsured patients.
