InGenesis is seeking an Urgent Care Registered Nurse to join our team in support of our client. In this essential role, you will collaborate with physicians and care teams to deliver patient-centered care for a diverse, often underserved population. As an urgent care registered nurse and physician extender, you'll assess symptoms, coordinate care, and guide patients to appropriate services via phone, video, or secure messaging—helping reduce emergency visits, ease provider workload, and improve outcomes. Key Responsibilities Conduct telephone or video-based assessments to evaluate patient symptoms and determine urgency of care for adult and pediatric patients using standardized clinical protocols. Utilizing critical thinking to assign triage levels, provide evidence-based recommendations, and refer to appropriate care settings (e.g., clinic appointment, urgent care, ED) Identify urgent cases and facilitate timely provider follow-up, same-day appointments, or emergency services as needed. Document triage assessments, nursing interventions, and patient instructions accurately in the electronic health record (EPIC) Collaborate with physicians, advanced practice providers, and care teams to coordinate timely follow-up Escalate high-risk situations to clinical leadership as appropriate Educate patients on chronic disease management, medications, and health promotion Participate in quality improvement initiatives and regular staff meetings via video conference Maintain current knowledge of clinical protocols and community health resources Provider Extender Support: Act as a clinical extension of the provider by: Conducting pre-visit chart prep Gathering histories and vitals Assisting in procedures Communicating provider care plans to patients Provide nursing interventions per standing orders and provider direction (e.g., medication administration, wound care, immunizations). Monitor labs, diagnostic results, and medication refills, and follow up with patients as directed by the provider. Care Coordination and Panel Management: Collaborate with case managers, behavioral health, dental, and pharmacy staff to coordinate wraparound services. Participate in daily huddles and multidisciplinary case reviews to ensure continuity of care. Assist in outreach and care gap closures (e.g., immunizations, cancer screenings, chronic disease management). Patient Education and Advocacy: Deliver culturally sensitive education on medications, chronic conditions, and preventive care. Empower patients to participate in shared decision-making and self-care, particularly those with complex or chronic conditions. Quality Improvement & Population Health: Contribute to UDS and HEDIS measure performance through proactive panel management. Help identify patients due for follow-up based on registries or risk stratification tools. Support implementation of evidence-based workflows to improve access and health outcomes.
