About The Position

Reporting to the Clinical Care Manager, the Triage RN is responsible for identifying, prioritizing, and coordinating patient/family needs for the purpose of providing preventative and supportive care to the terminally ill patient/family unit with the collaboration of the interdisciplinary team. This position is primarily performed in office, Monday - Friday 8am - 5pm. Work environment may be stressful at times, as overall activities and work levels fluctuate.

Requirements

• Currently licensed in the State of Florida as a registered nurse
• Minimum of two (2) years nursing experience

Nice To Haves

• Hospice, oncology, or home health nursing experience preferable
• Minimum of one (1) year experience in an acute care unit is desirable
• Minimum of one (1) year experience in caring for terminally ill patients is desirable
• Should have training in palliative care and symptom management

Responsibilities

• Attends to patient/family complaints and concerns
• Takes a proactive approach to identifying and resolving potential issues
• Educates the community regarding hospice care, philosophy, and concept through individual consultation with persons in the community
• Performs other duties as requested by the Clinical Care
• Provides assessment of the physical, psychological, social, and spiritual needs of the patient/family unit and initiates appropriate intervention
• Determines the classification of the phone call and notifies appropriate team and/or discipline
• Provides follow-up for information related to patient care issues and continuity of care to the appropriate CCM/RN Case Manager
• Receives telephone calls during business hours
• Accesses and supplements patient data via current computer program
• Communicates pertinent information to the team and necessary disciplines to ensure continuity of care
• Demonstrates knowledge of Gulfside Hospice policies and procedures and pertinent federal and state regulations
• Ensures professional standards of nursing practice are maintained
