About The Position

Join ameriCARE Chicagoland West as an RN Supervisor, providing compassionate and reliable support to clients in the Chicagoland area. This role is ideal for individuals dedicated to improving others' quality of life through personalized care and attentive assistance. Description: The RN Supervisor is responsible for assessing and reassessing the home care needs- including the physical, psycho-social, and environmental needs- of designated clients and their families. The RN shall be the designated supervisor to all care providers performing MF/MC services by making visits to each client’s place of residence at least every 92 days. When designated appropriate, the RN will also supervise all care providers performing CNA/PCA and Companion level services by making visits to each client’s place of residence at least every 92 and 122 days respectively. The RN shall document all supervisory visits. The RN shall also conduct assessments of the client’s condition, progress and response to services and document these visits in the client’s service record. Such assessments will be performed every 92 days for CNA/PCA level clients, and every 122 days for Companion level clients. The RN develops an appropriate and individual care plan, collaborating with the client’s primary physician as needed, and is responsible for teaching/counseling clients regarding self-care and evaluating client response to care provided. Acting as the clinical care coordinator for assigned individuals/ families, the RN initiates referrals as necessary, supervises and directs CNA/ PCA’s, Companions (where designated), families, and other caregivers regarding client care.

Requirements

  • Current unrestricted State Registered Nurse license and CPR certification.
  • Graduate of an accredited nursing program.
  • At least two years of work experience in home health care or nursing home care within the last 5 years.
  • Ability to meet agency’s bonding criteria. This includes never having been shown by credible evidence to have abused, neglected, sexually assaulted, exploited, or deprived any person or to have subjected any person to serious injury as result of intentional or grossly negligent misconduct as evidenced by the oral or written statement to this effect obtained at the time of application.
  • Valid driver’s license, available personal vehicle, and proof of current auto insurance.
  • Concise verbal, written, and organizational skills.
  • Five years of verifiable employment history, or a complete employment history if employment hasn’t exceeded five years.
  • Documentation of a satisfactory TB screening test upon employment and annually thereafter.
  • Demonstrated ability to effectively communicate and interact with families, co-workers, and all supervisors. Including ability to proficiently follow and process written and verbal instructions in the English language.
  • Hearing and visual capacities sufficient for accurate communication with all clients and co- workers, including assessment and recording of clinical information.

Nice To Haves

  • Extensive assessment skills.
  • Experience working with Joint Commission standards.
  • At least two years of nursing experience, and ACLS certification.
  • At least two years of work in home care industry.

Responsibilities

  • Supervise all CNA/PCA providers by making supervisory visits to each client’s place of residence at least every 92 days and provide written documentation of all such visits.
  • As part of this supervision, the RN will never knowingly permit an employee who has been exposed to tuberculosis or hepatitis or diagnosed with the same to provide services to clients until it is determined that the employee is not contagious.
  • Conduct and document competency demonstrations of all CNA/PCA providers at least annually, on the job tasks the staff member is required to perform.
  • Adhere to agency infection control and safety procedures by performing and documenting nursing observations, procedures, and interventions in the home according to the established care plan in a safe and competent manner.
  • Effectively communicate and interact with clients/caregivers and families in a supportive manner.
  • Management of client caseload and associated or other delegated work responsibilities in an effective manner within the resources of the agency and according to agency standards.
  • Interpretation of programs and policies as appropriate to clients and members of the community.
  • Participates in the continuing commitment for quality care, including attendance at mandatory agency staff meetings.
