Bella Terra Home Health & Hospice is seeking an experienced and compassionate Registered Nurse (RN) to join our clinical care team. In this role, you will plan, organize, and coordinate home health and hospice services in the patient’s home, ensuring patients and families receive high-quality, patient-centered care across the continuum of illness, including restorative, chronic, and end-of-life care.

Requirements

  • Active California RN License
  • Minimum 2 years of nursing experience (home health or hospice preferred)
  • Current CPR certification
  • Valid driver’s license and reliable, insured vehicle
  • Strong clinical, organizational, and communication skills
  • Ability to work independently and prioritize workload

Responsibilities

  • Perform comprehensive assessments and develop, implement, and revise individualized home health and hospice care plans in collaboration with patients, caregivers, and physicians
  • Provide skilled nursing care, including preventive, restorative, palliative, and rehabilitative interventions appropriate to home health and hospice patients
  • Manage symptoms, pain, medications, and treatments in accordance with physician orders and agency policies
  • Educate patients and families on disease processes, care plans, safety, symptom management, and end-of-life support when applicable
  • Document accurate, timely clinical notes and maintain compliant patient records per regulatory and agency standards
  • Coordinate care transitions, including admissions, recertifications, discharges, and hospice transitions, ensuring continuity of care
  • Communicate patient status, changes in condition, and care needs to physicians and the interdisciplinary team (IDT)
  • Update care plans and obtain physician orders as needed for both skilled home health and hospice services
  • Collaborate with interdisciplinary team members, including therapists, social workers, chaplains, aides, and community resources, to support holistic patient care
  • Participate in quality assurance, performance improvement programs, and ongoing in-service training
  • Provide clinical supervision and guidance to LPNs/LVNs and Home Health Aides, as applicable
  • Participate in on-call rotation to support patient needs after hours, weekends, and holidays as required
