Requirements

  • Active Registered Nurse (RN) license in the state of Illinois
  • Graduate of an accredited nursing program
  • Minimum 1 year of nursing experience (Home Health or Hospice experience preferred)
  • Current CPR certification
  • Knowledge of state and federal home care regulations
  • Strong clinical, organizational, and communication skills
  • A compassionate, patient-focused mindset

Nice To Haves

  • Home Health or Hospice experience preferred

Responsibilities

  • Deliver in-home nursing care to patients with acute, chronic, or end-of-life needs
  • Perform assessments, monitor patient conditions, and ensure accurate documentation
  • Administer medications and treatments as prescribed
  • Collaborate with physicians and an interdisciplinary care team
  • Educate and support patients and families on care plans, medications, and disease management
  • Provide compassionate, patient-centered care that upholds dignity and comfort
