Requirements
- Active RN license
- 2–3 years nursing experience (home health preferred)
- Strong clinical and communication skills
- EHR experience
- Valid driver’s license and reliable transportation
Responsibilities
- Perform in-home nursing assessments
- Develop and manage patient care plans
- Provide skilled nursing care including wound care, medication management, infusion therapy, catheter care, and palliative care
- Educate patients and caregivers
- Coordinate care with physicians and interdisciplinary team
- Document care accurately in EHR
- Follow safety and infection control standards
