About The Position
We are seeking a compassionate, dedicated Registered Nurse to provide high-quality in-home care to our patients. As a Home Health Nurse, you’ll deliver individualized care, support patient independence, and build meaningful relationships with those you serve.
Requirements
- Active RN license in Kansas
- Strong communication and clinical assessment skills
- Reliable transportation and valid driver’s license
- Compassion, professionalism, and a commitment to quality care
Nice To Haves
- Home health experience preferred but not required (training available)
Responsibilities
- Conduct in-home assessments and develop patient-centered care plans
- Provide skilled nursing care (medication management, wound care, monitoring chronic conditions, etc.)
- Educate patients and families on disease management and safety
- Document care accurately and timely according to agency standards
- Collaborate with an interdisciplinary team to coordinate care
